移植物抗宿主反应
移植物抗宿主反应 (graft versus host reaction, GVHR) 是由移植物中抗原特异性淋
巴细胞识别宿主组织抗原而发生一种排斥宿主组织抗原免疫反应, 常见于骨髓、胸
腺、脾等免疫器官移植或大量输血后。其发生条件是: ①移植物必须含有免疫活性细
胞; ②供、受者间组织相容性不合; ③受者无力发动摧毁移植细胞有关反应。移植物
抗宿主反应*常发生于同种骨髓移植后, 所造成对宿主损伤称为移植物抗宿主病
(graft versus host disease, GVHD) , 其主要临床表现是炎症性疾病, 患者可出现皮疹、腹
泻、肺炎等。除 MHC - Ⅰ类和Ⅱ类抗原不符外, 次要组织相容性抗原不符也可导致移
植物抗宿主病。
骨髓移植物中成熟 T 细胞是介导移植物抗宿主反应主要效应细胞, 细胞因子网络
失衡可能是造成移植物抗宿主反应重要原因。供者 CD4+T 细胞识别宿主组织相容性
抗原, 发生活化、增殖、分化, 产生 IL - 2, IFN -γ, TNF -α等细胞因子, 进一步激活
Tc 细胞、巨噬细胞、NK 细胞, 以直接或间接杀伤宿主靶细胞。免疫抑制剂对移植物抗
宿主反应治疗效果不佳。 |
