人工主动免疫
(一) 灭活疫苗 (死疫苗)
灭活疫苗 (inactivated vaccine) 是选用免疫原性强病原体, 经人工大量培养后,
用理化方法灭活制成。死疫苗主要诱导特异抗体产生, 为维持血清抗体水平, 常需多
次接种。注射局部和全身反应较重。由于灭活病原体不能进入宿主细胞内增殖, 难
以通过内源性途径加工、提呈抗原, 故不能诱导 CD8+T 细胞成为效应性 Tc 细胞, 免疫
效果有一定局限性。
(二) 减毒活疫苗
减毒活疫苗 (live - attenuated vaccine) 是减毒或无毒力活病原微生物制成生物
制品。传统制备方法是将病原体在培养基或动物细胞中反复传代, 使其失去毒力, 但
保留免疫原性。例如, 用牛型结核分枝杆菌在人工培养基上多次传代后制成卡介苗; 用
脊髓灰质炎病毒在猴肾细胞中反复传代后制成活疫苗。活疫苗接种类似隐性感染或轻症
感染, 病原体在体内有一定生长、繁殖能力, 一般只需接种一次。多数活疫苗免疫
效果良好、持久, 除诱导机体产生体液免疫外, 还可产生细胞免疫, 经自然感染途径接
种还可形成黏膜局部免疫 (表 26 - 1)。其不足之处是疫苗有体内回复突变危险, 但在实践中十分罕见。免疫缺陷者和孕妇一般不宜接受活疫苗接种。
(三) 类毒素
类毒素 (toxoid) 是用细菌外毒素经 0 . 3% ～0 . 4%甲醛处理制成。因其已失去外毒素毒性, 但保留免疫原性, 接种后能诱导机体产生抗毒素。 |
